Our in-home support services include personal care, domestic support, meal preparation, gardening, home maintenances, home modifications, in-home respite care, nursing care, transportation, allied health and other services.
We are attentive to your needs and provide three home care package options with the following cost effective structure:
1. Self-Managed - Package Management Fee 10% - Care Management Fee 0 %
Your choice of Self-Managed Home Care package includes the basic service listed below:
Administration / Package Management
Service Coordination and Rostering
Thorough home risk assessment
An annual care plan
Initial budget plan
You can enjoy the full independence to have full control of your care management needs. If you require additional Care Management service, the hourly rate of the Care Management Fee will apply.
2. Partnered Care Management - Package Management Fee 10% - Care Management Fee 6 %
Your choice of Partnership Care Management includes all the basic services in the Self-Managed option, plus regular review of your care needs by our professional Care Manager at minimum cost.
This means the majority of your home care subsidy will be spent on direct care needs or with in-home support services. You will enjoy the allocated time from our Care Management Team to listen and discuss your home care needs.
3. Full Support Care Management - Package Management Fee 10% - Care Management Fee 10 %
Hassle free unlimited Case Management service that gives you a full level of care and support. This choice is suitable for those who would like their care management looked after by our professional team.
4. Supreme Care Management - Package Management Fee 10% - Care Management Fee 18 %
The difference between the Supreme Care Management and other care Management plans is that in addition to all basic services, we arrange a dedicated care manager to provide you with high-quality management and tracking services throughout the process. In addition, another major feature of this plan is that the Active Community Group gives exclusive privileges to every senior who participates in this plan throughout the year.
