IN just five questions Queenslanders can have their say on the state's disability services.
The Towards an all abilities Queensland consultation survey is designed to help shape Queensland's future disability plans.
Disability Services Minister Coralee O'Rourke encouraged all Queenslanders to spare 10 minutes to complete the survey.
"The shorter survey has been designed with five simple questions, making it even easier for people to contribute their ideas on how we can better include, value and empower people with disability," Mrs O'Rourke said.
The consultation deadline has also been extended by two weeks until February 24.
"We are focused on creating a better future for all Queenslanders, but we can't do this alone," Mrs O'Rourke said.
"That's why we want to hear your views on how best to drive change to ensure our state is more inclusive for people with disability."
More than 70 submissions have already been received.
Submissions close February 24.
